How to Get Your Medical Records- Vanderbilt Health

This handout tells you how to get a copy of your or your child's medical records (younger than 18 years). 

There are two ways to get records online

You can go to: swellbox.com/vanderbilt-umc-wizard.html. Or make a request through My Health at Vanderbilt.

You'll need:

• your photo ID or driver's license (or a clear picture of your ID that you can upload)
• mobile phone or tablet
• providers names, types of records you need, location, and dates of service.

If you need your records sent somewhere outside of Vanderbilt Health, this is what you'll need

• name of the provider or facility
• full address
• phone number

If you need records for another patient besides your own child, this is what you'll need

• power of attorney document
• guardianship papers
• executor's documents
• death certificate or birth certificate of the patient
• other papers that show you're the legal representative.

Please note: the online request for medical records is for patients and legal representatives only.

You can also get your records with an authorization form

Do these things:

1. Download the authorization form at: VanderbiltHealth.com/information/medical-record-information. 
2. Fill it out.
3. Fax or mail the form to the place listed below where you or your child got their care.
